Is adrenal fatigue a factor?

New research indicates there is no such thing as 'adrenal fatigue' or adrenal exhaustion. However, we do know that your adrenal glands do work overtime when your body is under stress. This relationship is called the 'neuroendocrine' system. Neuroendocrine influences also contribute to Poly Cystic Ovarian Disorder (PCOS), Endometriosis, Fibroids and Thyroid disease. This is a complex set of direct and indirect feedback interactions that influence fertility in four ways:

  1. The HPA axis (hypothalamic–pituitary–adrenal axis) and fertility

Stress influences the HPA axis. This influences fertility and is connected to mood disorders and functional illnesses, including anxiety disorder, bipolar disorder, insomnia, post-traumatic stress disorder, borderline personality disorder, ADHD, major depressive disorder, burnout, chronic fatigue syndrome, fibromyalgia, irritable bowel syndrome, and alcoholism

  1. HPG axis (hypothalamic–pituitary–gonadal axis) and fertility

Stress influences the HPG axis, which refers to the hypothalamus, pituitary gland, and gonadal glands. These play a critical part in the development and regulation of the reproductive and immune system. The HPG axis controls Gonadotropin-releasing hormone (GnRH) influences the key fertility hormones; luteinizing hormone (LH), follicle-stimulating hormone (FSH), estrogen and testosterone.

  1. HPT axis (hypothalamic–pituitary–thyroid axis)and fertility

Stress influences the HPT axis. This influences ovulation, implantation and sperm quality. It is part of the neuroendocrine system responsible for the regulation of metabolism using the hypothalamus, pituitary gland, and the thyroid gland. It regulates thyroid hormone (Triiodothyronine (T3) and Thyroxine (T4)) and responds by releasing thyrotropin-releasing hormone (TRH) and thyroid-stimulating hormone (TSH). If this is part of the problem, you may have irregular cycles of ovulation or may not be ovulating at all. It's possible you have had several miscarriages or poor sperm health may be a problem.

  1. Hypothalamic–neurohypophyseal system and fertility

Stress influences the HPN system.  This influences the pituitary which influences ovulation and other key components of fertility.

Combined, these 4 influences highlight why you should not ignore fatigue and not take supplements or herbal formulas without getting to the cause first.

It’s easy to see fatigue plays a large role in why you can’t conceive and contributes to female health problems like PCOS, Endo, and Fibroids. Fatigue symptoms provide important clues for what you need to do to improve your fertility for when you are ready to have a baby.

So how does this loop influence male fertility?

In males, FSH stimulates primary spermatocytes (a phase of sperm’s development) leading to the maturation of sperm cells. 

LH stimulates the production of the sex hormones (androgens) by the Leydig cells of the testes. The most widely-known androgen in males is testosterone, which promotes the production of sperm and masculine characteristics. 

So how does this loop influence male fertility?

In females, FSH stimulates the development of egg cells (or ova) in structures called follicles. LH also plays a role in the development of ova, induction of ovulation, and stimulation of estradiol (a type of estrogen) and progesterone production by the ovaries.

If these processes are not triggered by the GnRH, the development of sperms and follicles is dysfunctional. Without ovulation (the condition is called anovulation) and sperm production, the pregnancy is not possible.

Is it possible to reverse infertility caused by fatigue?

Fatigue usually does not present overnight but has a gradual onset which means the longer you have felt this way, the more important it is for you to start working on it.  

It is important to remember that fertility problems caused by fatigue are often modifiable and reversible. The most important thing you need to do is to get to the cause.
